Wolfgang Miesbach: FRONTIER4 Long-Term Results Highlight the Promise of Denecimig in Haemophilia A
Wolfgang Miesbach, Professor of Medicine at Frankfurt University Hospital, shared a post on LinkedIn:
”FRONTIER4 — denecimig (Mim8) long-term data presented at ISTH 2026 in Paris.
Among the clinically relevant prophylaxis updates, OC 02.4 by Jerzy Windyga reported interim safety and efficacy results from FRONTIER4, the long-term extension study of the denecimig (Mim8) programme in haemophilia A.
Denecimig is a next-generation subcutaneous FVIIIa mimetic, optimised for FIXa cofactor activity and thrombin generation.
What sets the programme apart is the dosing flexibility: once weekly, once every two weeks, or once monthly — by patient choice after week 26.
Interim analysis in adults and adolescents (≥12 years, N=365; median observation ~0.5 years):
Efficacy
- Overall estimated mean ABR 0.75, consistently low across all three schedules (QW 0.91, Q2W 0.55, QM 0.74)
- Median observed ABR was zero in every subgroup
- 71.2% with zero treated bleeds
- Low spontaneous bleeds (ABR 0.09) and near-absent target-joint bleeds (ABR 0.01, 97% zero)
- Low ABRs irrespective of FVIII inhibitor status (positive 0.15, negative 0.88)
Safety
- No thromboembolic events
- Injection-site reactions in 1.8% of injections (108/6138); the majority of AEs mild
- No clinical evidence of neutralising anti-denecimig antibodies
Monthly dosing maintained bleed protection comparable to weekly — a genuine opportunity to reduce treatment burden, with low bleed rates holding regardless of inhibitor status.
