“New Publication in Case Reports in Cardiology

We look at a severe complication in the cath lab:

a 67 year old patient who developed Type II Heparin Induced Thrombocytopenia (HIT) following a single diagnostic heparin bolus during a routine angiogram.

Within hours, the patient suffered an acute arterial thrombotic storm spanning multiple vascular beds imb ischemia, inferior STEMI, and ischemic stroke.

A few practical reminders from this case:

  • Severe arterial thrombosis can hit well before platelets drop below normal thresholds. A 50 percent relative drop is the real red flag.
  • Post bolus acute distress is easily misdiagnosed as contrast allergy, delaying the crucial decision to stop heparin.
  • When clinical suspicion (4T score) is high, don’t wait on functional assays switch immediately to non heparin anticoagulants.”
