Amirali Ebrahimpour: Escherichia coli–Related DIC Case Report

Amirali Ebrahimpour, Member of the Educational Development Center (EDC) Task Group at Varastegan Institute for Medical Sciences and Teacher Assistant in Practical Hematology, Pharmacology, Mycology, and Biochemistry, shared on LinkedIn:

“Today, I had the opportunity to present a detailed clinical case report on one of the most critical complications associated with severe bacterial infections: Escherichia coli–related Disseminated Intravascular Coagulation (DIC).

The case focused on a 48-year-old patient who developed acute gastroenteritis after consuming undercooked cured meat, ultimately progressing to sepsis and DIC. This clinical scenario underscores the importance of rapid recognition and timely intervention—especially through early monitoring of platelet count, coagulation abnormalities, procalcitonin levels, and multi-organ function.

In reviewing this article, I highlighted several essential takeaways:

  • Sepsis can rapidly trigger coagulation dysregulation, leading to life-threatening DIC
  • Thrombocytopenia, elevated D-dimer, and prolonged PT are key diagnostic indicators
  • Procalcitonin plays a valuable role in assessing bacterial infection severity and guiding antibiotic duration
  • Early administration of broad-spectrum antibiotics (such as meropenem) combined with appropriate anticoagulation can significantly improve prognosis
  • With timely and targeted therapy, even severe cases can achieve full recovery—as demonstrated in this report

Overall, this case reinforces the critical value of early diagnosis, biomarker-based monitoring, and evidence-driven treatment strategies in managing severe infections and preventing fatal complications such as DIC.

Special thanks to my colleague Yeganeh Khazaei for collaborating with me in presenting this case.”
