Cedric Hermans: What If Higher Plasma FIX Levels Do Not Necessarily Mean better Haemostatic Protection?

Cedric Hermans, Co-Chair FVIII/FIX SSC ISTH 2025, shared a post on LinkedIn about recent article he and his colleauges co-authored, published in JTH, adding:

“What if higher plasma FIX levels do not necessarily mean better haemostatic protection?

In a recent commentary published in the Journal of Thrombosis and Haemostasis, we discuss the important findings of Davide Matino and colleagues on the influence of endogenous FIX antigen (CRM status) on the pharmacokinetics of extended-half-life FIX concentrates.

Their findings challenge our conventional understanding of FIX pharmacokinetics and highlight the potential importance of extravascular distribution.

The key message?

Understanding FIX replacement therapy requires looking beyond plasma FIX levels to consider distribution, tissue binding, and the patient’s endogenous FIX biology.

An important step towards more individualized treatment of haemophilia B.

Our commentary.”

Title: Factor IX gene variants may influence the pharmacokinetic profile of patients with hemophilia B treated with Factor IX-fusion protein or N9-GP extended half-life Factor IX concentrates
