ICAS Not a Factor: Post-EVT Intensive BP Control within 24 hrs Worsens Outcomes

International Journal of Stroke (IJS) shared a reminder post on X:

”Intensive blood pressure lowering in acute stroke with intracranial stenosis post-thrombectomy: A secondary analysis of the OPTIMAL-BP trial.”

IJS is here to remind us about the important from a ‘Intensive blood pressure lowering in acute stroke with intracranial stenosis post-thrombectomy: A secondary analysis of the OPTIMAL-BP trial‘ by Kwang Hyun Kim et al.

Asian randomized trials indicate that intensive blood pressure (BP) control within 24 hours after successful endovascular thrombectomy (EVT) is associated with poorer functional outcomes compared to conventional BP management.

Despite the high prevalence of intracranial atherosclerotic stenosis (ICAS) in this population, ICAS did not significantly modify the effect of intensive BP lowering.

These findings suggest that intensive BP management should be avoided post-EVT regardless of ICAS status to optimize recovery.
