Asian randomized trials indicate that intensive blood pressure (BP) control within 24 hours after successful endovascular thrombectomy (EVT) is associated with poorer functional outcomes compared to conventional BP management.

Despite the high prevalence of intracranial atherosclerotic stenosis (ICAS) in this population, ICAS did not significantly modify the effect of intensive BP lowering.

These findings suggest that intensive BP management should be avoided post-EVT regardless of ICAS status to optimize recovery.
