Jim Meliezer: Aspirin May Help Prevent Recurrent Preterm Birth
Jim Meliezer, Arts-onderzoeker in Amsterdam UMC shared a post on LinkedIn about a recent article by James A. L. Meliëzer et al, published in BJOG, adding:
“Our paper is out!
In this secondary analysis of the April trial, we looked at the placenta of the previous spontaneous preterm birth.
The APRIL trial showed a non significant reduction in recurrent preterm birth when women used low dose aspirin after a previous preterm birth.
In our analysis, low dose aspirin in the next pregnancy was associated with far fewer recurrent preterm births in women whose previous placenta showed signs of maternal vascular malperfusion (MVM): 4 percent (1 24) with aspirin vs. 45 percent (9 20) with placebo.
In women without MVM, we saw no benefit.
As a post hoc analysis with small numbers, these findings are hypothesis generating and need confirmation in a prospective trial.
Still, they support the idea that spontaneous preterm birth is not one condition, but the outcome of several underlying causes.
Placental histology may help identify a vascular mediated subgroup that could benefit from aspirin.
Thank you to all co authors, the APRIL study group, the participating centres and, of course, all participants!”
