Manuel Requena Shares The Results of the RESISTANT International Registry

”I am pleased to share the results of the RESISTANT International Registry, recently published in Stroke: Vascular and Interventional Neurology, evaluating intracranial stenting during acute endovascular therapy for ischemic stroke.

I am deeply grateful to all the investigators, interventional teams, and stroke centers worldwide whose collaboration, commitment, and expertise made the RESISTANT registry possible.

This study reflects the power of international collaboration in advancing stroke care.

Together, we collected the largest cohort of patients who underwent acute intracranial stenting (876 patients across 36 international centers).

Key findings:
1. Acute intracranial stenting proved feasible and effective, achieving successful reperfusion (eTICI 2b–3) in nearly 90% of cases.
2. 41% of patients achieved good functional outcomes (mRS 0–2) at 90 days, despite the complexity of these challenging scenarios.
3. Final successful reperfusion and a lower number of thrombectomy attempts before stenting were the strongest predictors of favorable outcomes.
4. Reocclusion occurred in a minority of cases, mostly within the first 48 hours, and no independent predictors of symptomatic intracranial hemorrhage were identified.
