New Researchs at ACC26 on Everyday Clinical Decisions – The Lancet Group

“Cardiologists – The Lancet is at ACC26 with new research exploring everyday clinical decisions.

1.Aspirin vs clopidogrel after PCI:

Longest randomised follow-up (ten years) suggests sustained reductions in clinical events with clopidogrel compared with aspirin after PCI:

  • Lower ischemic events with clopidogrel
  • Lower bleeding events with clopidogrel
  • No difference in mortality

‘These findings suggest that clopidogrel may be considered as the preferred agent for long-term antiplatelet monotherapy during the chronic maintenance phase after PCI’, say authors.

2.PCI before TAVI—necessary?

In patients with CAD, deferring PCI was noninferior to a PCI-first strategy at 1 year for death, MI, stroke, and major bleeding.

‘These findings support an initial conservative approach, although patient-tailored treatment strategies remain essential’, say authors.”
