Vera Ghali: 2026 ASH–ISTH Recommendations on Pediatric VTE Prevention
Vera Ghali, Member of the Lebanese Order of Physicians and Lebanese Pediatric Society, shared Blood Journals Portfolio‘s post on X:
“The 2026 ASH-ISTH guidelines and recommendations on anti-coagulant prophylaxis for pediatric patients at risk of Venous Thromboembolism (VTE).
The panel underscores the necessity to individualize anti-coagulant prophylaxis for pVTE and resorting to careful assessment of each child’s thrombotic and bleeding risks, with special attention to children’s specificities across different subgroups.
The panel highlights that children with leukemia and lymphoblastic lymphoma (LBL) who benefit the most from anticoagulation are those who are older than 10 years, those with obesity, T-cell immunophenotype, high-risk Acute Lymphoblastic Leukemia (ALL), and children with a personal or family history of thrombosis.
Factors that increase bleeding with anticoagulant prophylaxis are: younger age, severe thrombocytopenia, renal dysfunction, and previous bleeding.
Furthermore, asparaginase is a severe pro-thrombotic agent, consequently, anticoagulation should be initiated during asparaginase-containing chemotherapy cycles and discontinued as soon as the asparaginase hypercoagulable effects wear off.”
