”New NEJM Clinical Practice review on PAD in the legs (McDermott).
A really practical synthesis that reinforces a few key points:
- PAD remains common and functionally limiting, with diagnosis still anchored around ABI <0.90. My take: Using quantitative Doppler waveform analysis.
- Event prevention is ‘full-spectrum’: intensive lipid-lowering (statins), antiplatelet therapy / low-dose aspirin + rivaroxaban (where appropriate), BP <130/80, and in relevant patients GLP-1RA (semaglutide) and SGLT2 inhibitors.
- For symptoms, supervised or structured home-based walking exercise remains foundational; revascularisation is best reserved for persistent symptoms not responding to exercise.
- Worth a read for anyone building PAD pathways across primary care/CDC and vascular services.”
