”Night Sweats: Not Always a Haematological Plot Twist
One of the most common reasons patients end up in our clinics is the humble night sweat – the kind that soaks the sheets and sparks a full WebMD-fuelled existential crisis by morning.
But here’s the twist: most night sweats have absolutely nothing to do with haematological malignancy.
(Shockingly, not everything is lymphoma… though we all secretly enjoy the diagnostic puzzle.)
Night sweats can spring from a whole spectrum of causes: infection, endocrine issues, neurological conditions, connective tissue disorders, menopause, sleep apnoea, diabetes – you name it. Haematological causes sit quietly in the corner, waving politely.
Useful initial tests include:
- FBC and blood film
- ESR / CRP
- TFTs
- Glucose
- Viral serologies (EBV/CMV/HIV/Hep)
- Immunoglobulins
- CXR
- Lymph node and spleen examination
When to refer:
- Systemic red flags (weight loss, fevers, alcohol-induced pain)
- Lymphadenopathy or splenomegaly
- Abnormal FBC (always worth a chat with your friendly haematologist)
Who doesn’t need referral?
Patients with only night sweats and a normal FBC
