Omar Adwan: Systematic Approach to Blood Smear Examination
Omar Adwan, Medical Laboratory Technologist at Modawah lab center, shared a post on LinkedIn:
”Blood Smear Examination (Summary of Steps)
Start with basic cell parameters
- Review WBC, Platelet, and RBC values from the Automated Hematology Analyzer (Count, MCV, MCH, MCHC, RDW, Hb, Hct).
- Use these parameters to predict possible abnormalities:
Elevated WBC may indicate a possible infection or leukemia
Low platelets increase the risk of bleeding
Abnormal RBC may indicate anemia or an RBC disorder
Evaluate Red Blood Cell Morphology (RBC Morphology)
- Examine the size, color, shape, and uniformity of RBCs.
- Main categories:
-Normocytic,
-Microcytic,
-Macrocytic,
-Normochromic/Hypochromic.
Important: Begin by assessing the RBC type based on parameter values to identify the predominant RBC morphology.
Then, look for additional RBC abnormalities observed on the smear.
Assessing RBC type first provides a framework for identifying other abnormalities.
Check for abnormalities in all cell types on the slide
- WBC: Immature forms, Blasts, Hypersegmented Neutrophils
- Platelets: Clumping, Giant Platelets
- RBC: Target cells, Spherocytes , Schistocytes , Anisocytosis
Summary:
- Start with parameters, then evaluate RBC type, and finally examine all abnormalities
- This method ensures a complete, accurate report consistent with the parameter findings.”
