Are We Underestimating VTE After PanNET Surgery? – RPTH Journal

RPTH Journal shared a post on LinkedIn about a recent article by Kelly Dong et al:

”Postoperative VTE in pancreatic neuroendocrine tumors. Are we underestimating risk?

A large NSQIP analysis (n=60,649) examined VTE after pancreatectomy for pNENs vs other histologies.

Key findings:

  • Overall VTE risk is similar to other pancreatic cancers (approximately 3.6%)
  • But DVT risk is higher in pNEN vs PDAC (adjusted OR approximately 1.25)
  • PE risk is not significantly different

What drives VTE in pNEN?

Independent risk factors include:

  • Increased BMI
  • Pre-op transfusion
  • Longer operative tim
  • Total pancreatectomy
  • Longer hospital stay

Interpretation:

Despite being perceived as ‘less aggressive’ tumors, pNEN patients carry a meaningful thrombotic burden, particularly for DVT.

This is clinically important because:

  • Guidelines for extended prophylaxis in pNEN are unclear
  • Tumor heterogeneity may mask high-risk subgroups
  • Surgical complexity still drives thrombosis risk”
