“Rusty leg syndrome in thalassemia major

Diverse cutaneous manifestations in thalassemia– seen in up to 90% patients:

  • Skin type – Fitzpatrick skin type- 3, 4 or 5
  • Bronze or slate-grey skin tone
  • Freckles
  • Xerosis cutis – with or without pruritis
  • Rusty leg syndrome or hemosiderin depositions
  • Pseudoxanthoma Elasticum (PXE)-Like Syndrome- oxidative stress and iron-mediated damage to elastic fibres- resembling ‘plucked chicken skin’
  • Chronic Leg Ulcers: 8% of patients, chronic tissue hypoxia, iron overload, and venous insufficiency
  • Cutaneous Small-Vessel Vasculitis (CSVV)
  • Drug-related – Chelators related- Skin rashes and localised reactions at the injection site, such as induration, scarring, or urticaria or Hydroxyurea-related- Melanonychia (nail pigmentation), skin ulcers, and alopecia.”
