Stuart T. Haines: Pharmacist-Led Warfarin Management Improves Therapeutic Control in HeartMate 3 LVAD Patients
Stuart T. Haines, Editor-in-Chief JACCP, Professor and Director of Professional Development at University of Mississippi School of Pharmacy, shared on LinkedIn about a recent article by Bailey M. Colvin et al, published in Journal of the American College of Clinical Pharmacy, adding:
”The evidence for pharmacist-managed anticoagulation in patients just got a lot stronger.
A new study published in the Journal of the American College of Clinical Pharmacy – the largest multicenter cohort focused exclusively on LVAD patients – found that pharmacist-led warfarin management achieved a mean time-in-therapeutic range (TTR) of 65.6% versus 45.8% in patients managed by nonpharmacist clinicians.
That near-20-point advantage held after multivariable adjustment and came with no significant increase in clinically significant bleeding.
This fills a real evidence gap.
Prior studies in LVAD populations were small, single-center, and often included older device generations.
This study’s multicenter design and exclusive focus on the HeartMate 3 LVAD reflect current clinical practice – and the results strongly support the key role that pharmacists can play in the care of this very complex patient population under collaborative practice agreements.
