Ahmed Kardousha: Protecting Growth and Fertility in Transfusion-Dependent β-Thalassemia
Ahmed Kardousha, Oncology and Hematology Pharmacist at National Center for Cancer Care and Research (NCCCR), shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in Indian Journal of Hematology and Blood Transfusion, adding:
“Excited to share our latest publication in the Indian Journal of Hematology and Blood Transfusion:
‘Growth and Endocrine Preservation in Transfusion-Dependent β-Thalassemia: Updated Pharmacologic and Genomic Interventions.’
This review explores one of the most challenging long-term complications in transfusion-dependent β-thalassemia (TDT): growth impairment and endocrine dysfunction caused by chronic anemia and iron overload.
Key findings from our review include:
- Despite advances in transfusion and iron chelation therapy, endocrine complications such as hypogonadism, growth failure, and metabolic dysfunction remain highly prevalent in adults with TDT.
- Emerging therapies including luspatercept, hydroxyurea, and thalidomide show promising benefits in reducing transfusion burden and improving metabolic stability in selected patients.
- Gene therapy and CRISPR-based genomic interventions demonstrated remarkable outcomes, with transfusion independence achieved in more than 90% of treated patients in several studies.
- Early intervention appears critical — timely disease modification may help preserve normal growth and pubertal development before irreversible endocrine damage occurs.
- The degree of iron reduction and hematologic correction were identified as key predictors for endocrine preservation.
Our review emphasizes the importance of integrating endocrine and fertility outcomes into future thalassemia trials and long-term patient care strategies.
