”AF plus CHA₂DS₂-VA Score – 0… yet I prescribed an Oral Anticoagulant!
Can you identify the reason?
There are several situations where patients with Atrial Fibrillation (AF) require anticoagulation regardless of their CHA₂DS₂-VA score
Hypertrophic Cardiomyopathy (HCM)
- Fontan circulation
- Systemic right ventricle
- Intracardiac cardiac repair in congenital heart disease
- Cyanotic heart disease
- After AF ablation (for 8 weeks)
- After cardioversion (for 4 weeks)
- Mechanical prosthetic valve
- Moderate or severe mitral stenosis
- Other indications for anticoagulation (DVT, PE, LV thrombus, etc.)
- Thyrotoxicosis (in selected recommendations)
- Cardiac amyloidosis
Remember: A low CHA₂DS₂-VA score does not always mean no anticoagulation!
