Salvador Payán Pernía: Ferritin Levels and Transferrin Saturation, Reflecting Rapid Iron Accumulation and Reduced Iron Utilization

Salvador Payán Pernía, Member of the Board of Directors of the Erythropathology Group at Spanish Society of Hematology and Hemotherapy (SEHH), shared on LinkedIn about a recent article he and his co-authord, published in Blood Advances, adding:

“I’m delighted to have contributed to this European multicentre study on acquired pure red cell aplasia (PRCA), one of the rare diseases I most enjoy managing.

Acquired PRCA is an underrecognized entity, even among hematologists, as reflected by the median time from presentation to treatment of 2 months (range: 1–204 months) observed in our study.

PRCA should be promptly suspected in any patient with severe anemia and a low reticulocyte count. In our study, earlier initiation of cyclosporine A was associated with a better response.

Our study also shows that PRCA is characterized by markedly elevated erythropoietin levels (median: 908 mIU/mL; range: 29–3000 mIU/mL), likely related to reduced degradation by erythroblasts.

These findings argue against a significant role for erythropoietin analogues in the management of PRCA.

At diagnosis, most patients had elevated ferritin levels and transferrin saturation, reflecting rapid iron accumulation and reduced iron utilization.

Reticulocytes are crucial not only for diagnosis but also as an early marker of response, as highlighted in the paper –look at the reticulocytes!
