Dynamics of platelet activation in cold-stored low-titer group O whole blood over 8 days: insights relative to standard platelet concentrates

As the use of low-titer group O whole blood (LTOWB) continues to expand in trauma and critical care, understanding the biology of platelets during refrigerated storage has become increasingly important.

Aim: To characterize the platelet activation phenotype of cold-stored LTOWB and compare it with standard platelet concentrates throughout 8 days of storage.

Key findings: LTOWB exhibits a distinct platelet activation profile, with progressive basal activation during cold storage while maintaining robust agonist-induced responses through Day 8.

Why does it matter?

These findings provide new insights into the biology of LTOWB and support future studies investigating how storage-related platelet changes may influence hemostasis, immunothrombosis, endothelial interactions, and transfusion-associated lung injury in hemorrhagic resuscitation.
