“Blood component therapy is a cornerstone in managing hematological disorders, offering targeted transfusion strategies to optimize patient outcomes.

This chapter explores the role of various blood components, including packed red blood cells (PRBCs), platelets, fresh frozen plasma (FFP), and cryoprecipitate, in treating anemia, thrombocytopenia, and coagulopathies.

PRBC transfusions focus on restrictive strategies to balance oxygen delivery while minimizing risks such as iron overload and transfusion reactions.

Platelet transfusions are essential in thrombocytopenia management, with strict guidelines to prevent transfusion refractoriness and alloimmunization.

FFP plays a critical role in coagulation factor replacement, particularly in disseminated intravascular coagulation (DIC) and massive transfusion protocols.

The chapter also highlights the complex transfusion needs of hematopoietic stem cell transplantation (HSCT) patients, emphasizing the importance of leukoreduction, irradiation, and ABO compatibility.

Special considerations for autoimmune hemolytic anemia (AIHA) and fetal anemia due to Rh isoimmunization are discussed, underlining the challenges in immunohematology.
