“Myeloproliferative Neoplasms, Review in New England Journal of Medicine (2026).
Key Points:
- JAK2, CALR, and MPL cause more than 90% of cases. Persistent thrombocytosis or erythrocytosis without a secondary cause: request molecular panel.
- The mutation precedes the disease by decades. A positive JAK2 does not equal a diagnosis.
- Thrombosis is driven by inflammation, not the count. In ET, stratify with IPSET Score, not with platelets.
- Polycythemia Vera: Pegylated interferon alfa is superior to hydroxyurea in preventing thrombosis.
- No current treatment eradicates the clone. JAK inhibitors improve symptoms; the survival benefit is modest. Anti-mutated CALR therapies remain in early phase.
- Transformation to AML: 0.7–3.8% in ET vs 20.6% in primary myelofibrosis. Once transformed, median survival 3–5 months.”
