Adil Aftab: Anticoagulation in CKD – Key Updates from the 2026 ESC Guidelines
Adil Aftab, Postgraduate Trainee at People’s University of Medical and Health Sciences for Women, Nawabshah, shared a post on LinkedIn:
“Anticoagulation in CKD: Key Updates from the 2026 ESC Guidelines
The 2026 ESC Guidelines for the management of cardiovascular disease and chronic kidney disease highlight the importance of individualized anticoagulation decisions in patients with chronic kidney disease (CKD) and either venous thromboembolism (VTE) or atrial fibrillation (AF).
- Renal function matters: Anticoagulant selection should be tailored according to eGFR and bleeding/thromboembolic risk.
- AF: Factor Xa inhibitors are generally preferred in patients with eGFR 30–59 mL/min/1.73 square meters.
- Advanced CKD: At eGFR below 30, treatment becomes increasingly individualized, with careful consideration of bleeding risk and available evidence.
- Dialysis: Decisions regarding anticoagulation require particularly careful risk–benefit assessment because evidence remains limited.
- VTE: Factor Xa inhibitors may be preferred over LMWH or VKA in selected CKD stages, while very advanced CKD requires individualized decision-making.
This guideline reinforces an important clinical principle: anticoagulation in CKD is not ‘one size fits all.’ Renal function, indication, thromboembolic risk, bleeding risk, and the specific anticoagulant all need to be considered.
A valuable update for clinicians involved in Internal Medicine, Cardiology, Nephrology, and Hematology.”
