Archil Jaliashvili: What if Normal Hb and SpO₂ Still Do not Mean Adequate Tissue Oxygenation?

“What if normal Hb and SpO₂ still do not mean adequate tissue oxygenation?

This new review raises exactly this question.

In severe inflammation and critical illness, the problem is not simply how much oxygen is present in the blood.

  • Endothelial dysfunction.
  • Microcirculatory impairment.
  • Edema.
  • Microthrombosis.
  • Altered NO biology.
  • Longer oxygen diffusion distance.

All of these can determine whether oxygen actually reaches the tissue.

This is why I believe we need to move beyond thinking only in terms of hemoglobin, saturation and macrocirculation.

We need a broader concept: Precision Oxygenation Therapeutics.

The therapeutic objective should not simply be to improve numbers in the bloodstream.

It should be to maintain effective oxygen availability where it is actually needed at the tissue level.

This also creates an important research question for BHOC and next-generation HBOCs.

Not as a replacement for RBCs.

But as a complementary oxygen-delivery platform that may help support tissue oxygenation when conventional RBC-dependent transport, endothelial

regulation and microcirculation become physiologically limited.

The future question may therefore be less:

‘How much hemoglobin do we have?’

and more:

‘How much oxygen is actually reaching the tissue and can we control this more precisely?'”
