Melissa Hollo: Pre-Operative Anemia – An Underrecognized Challenge in Surgical Care

“One in three patients undergoing major elective surgery has pre-operative anemia. What happens if we stop treating that as ‘normal’?
Tiffany Hall and I analyzed 50,033 surgical patients across 40 U.S. hospitals and found:
- 33.2 percent had pre-operative anemia
- Nearly half had moderate-to-severe anemia
- Prevalence reached 45.7 percent in women’s health and 52.4 percent in vascular surgery
- Women and older adults carried the greatest burden
These findings reinforce a critical opportunity in Patient Blood Management:
Don’t wait for the transfusion.
Identify and optimize the patient before surgery.
Pre-operative anemia is common, consequential, and often under-recognized.
Proactive identification and treatment should be part of the surgical pathway.
