“Sickle Cell Disease – The HbS Story
HbS is an abnormal hemoglobin caused by a single amino acid substitution in the β-globin chain:
Glutamic acid – Valine at position 6
When oxygen levels decrease, HbS polymerizes inside the RBC – causing sickling, hemolysis, and vaso-occlusion.
Peripheral smear may show:
- Sickle cells
- Target cells
- Polychromasia
- Nucleated RBCs
- Howell-Jolly bodies
HbSS pattern:
HbS predominant | HbA absent | HbF variable
ASCP Pearl:
Sickled cells and hemolytic anemia and no HbA – Think HbSS.
