“Beyond Efficacy: Why Medication Adherence and Drug Exposure Matter in Clinical Development
An important Yale-led study highlights how a well-established therapy may provide benefits beyond controlling the primary disease.
Researchers followed 248 patients with systemic lupus erythematosus and found that very low blood levels of hydroxychloroquine – together with poor medication adherencee – were associated with increased cardiovascular risk over one to two years.
The findings support therapeutic drug monitoring and more individualized dosing strategies rather than a uniform, one-size-fits-all approach.
From a clinical-trial and drug-development perspective, this study reinforces several important principles:
- Clinical outcomes depend not only on the prescribed dose but also on actual drug exposure and sustained adherence.
- Pharmacokinetic and adherence data can help explain variability in treatment response.
- Long-term studies should assess outcomes beyond disease activity, including cardiovascular and other organ-related complications.
- Real-world evidence can uncover additional value from established therapies and inform the design of future prospective trials.
- Patient education and point-of-care monitoring may themselves become meaningful components of treatment optimization.
This is a strong example of how clinical research can move an established medicine toward more personalized, measurable, and outcomes-focused care.”
