”When the CBC lies…
The platelet count looks low…
until you see the smear.
Have you ever received a CBC showing thrombocytopenia, only to look at the peripheral smear and realize that the analyzer may not be telling the whole story?
This is where platelet satellitism can become an important laboratory clue.
What happens?
Platelets may adhere to the surface of neutrophils in vitro, producing a characteristic ‘rosette’ or satellite-like appearance around the neutrophil.
Why does it matter?
- It is usually an EDTA-dependent phenomenon
- It can result in pseudothrombocytopenia on automated analyzers
- It may prevent unnecessary investigation or treatment for presumed true thrombocytopenia
- The peripheral smear can provide the clue that the analyzer cannot
Lab Tip:
When platelet satellitism is suspected, consider repeating the platelet count using a citrate tube or another appropriate anticoagulant, according to your laboratory protocol.
The important lesson?
‘Never interpret the platelet count completely in isolation.’
Sometimes, the most important information isn’t in the CBC report — it’s sitting right there on the smear.”
