Lorenz Räber: Intracoronary Imaging in PCI – Why, When, How, and Which Modality?
Lorenz Räber, Vice President of Swiss Society of Cardiology, shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in Nature Reviews Cardiology, adding:
“Intracoronary imaging in PCI: Why, when, how—and which modality? Insights from large randomized controlled trials
Our review has just been published in Nature reviews cardiology, part of Springer Nature
The publication of eight large international randomized controlled trials (each including more than 1,000 patients)—ILUMIEN IV, IVUS-ACS, IVUS-CHIP, IVUS-XPL, OCCUPI, OCTOBER, RENOVATE-COMPLEX, and ULTIMATE—has generated substantial evidence supporting the clinical use of intracoronary imaging (ICI) to improve outcomes in patients undergoing PCI.
However, heterogeneity in study populations, the lack of standardized criteria across ICI-guided PCI protocols, and mixed results across randomized trials have left important uncertainty regarding why, when, and in whom ICI guidance provides the greatest clinical benefit in daily practice.
In this 20-page review, we discuss all major randomized controlled trials of intracoronary imaging guidance—including the latest IVUS-CHIP and OPTIMAL—and provide a practical perspective on the optimal use of IVUS and OCT in contemporary PCI.
