Ifeanyichukwu Ifechidere, Specialist Biomedical Scientist at Sheffield Teaching Hospitals, shared a post on LinkedIn:
“What if I told you platelets don’t ‘stop bleeding’… at least not in the way most of us were taught?
We often learn primary hemostasis as a neat little sequence:
Vessel injury
- Platelet adhesion
- Platelet activation
- Platelet aggregation
- Plug formation
Simple, right?
But then the questions start getting uncomfortable…
Why does a platelet need to change shape?
Why does adhesion require more than just ‘platelets sticking to the damaged vessel’?
Why is von Willebrand factor so important in this process?
And perhaps the more interesting question:
If platelets are so good at forming a plug, why doesn’t every small vascular injury result in uncontrolled thrombosis?
Something has to regulate this process.
And the platelet is doing far more than simply ‘plugging the hole.’
There’s a fascinating conversation happening between the injured vessel, endothelial cells, von Willebrand factor, platelet receptors, agonists, granules and coagulation factors.
Miss one piece, and suddenly primary hemostasis doesn’t look quite as simple as the textbook diagram suggests.
So here’s my question:
Do we really understand what the platelet is doing during primary hemostasis—or have we just memorized the steps?
