Ahmed Bennis: Understanding the Diagnosis, Risk, and Management of ARN

Ahmed Bennis,  Professor of Cardiology at Ibn Rochd University Hospital, shared a post on X about a recent article by Yiwen Chen et. al, published in Clinical Kidney Journal, adding:

“Recent advances in anticoagulant-related nephropathy: risk factors, peribiopsy and therapeutic anticoagulation.

Definition: AKI or worsening of CKD caused by anticoagulant use. First proposed in 2009, with more than 140 cases reported by 2025.

Risk factors: CKD, IgA nephropathy, advanced age, comorbidities.

Diagnosis: Needs renal biopsy to exclude other causes. Hallmarks are extensive intratubular RBCs and/or RBC casts and ATN, without active glomerulonephritis.

Treatment: Supportive, with restoration of target anticoagulation.

Peribiopsy anticoagulation depends on the thrombotic risk of the primary disease and the characteristics of the anticoagulant.

ARN is linked to poor survival and kidney outcomes.

Evidence is case-level, so individualize care.”
