Tooth Extraction in Patients with Haemophilia – RPTH Journal

“Tooth extraction in haemophilia

Delayed bleeding is more common than we might think.

Tooth extraction in patients with haemophilia requires careful planning, but even with specialist care, postoperative bleeding remains a real risk.

A new retrospective multicentre study in RPTH Journal from Japan provides valuable real-world data on this underexplored topic.

Key findings from 271 procedures in 198 patients with haemophilia A or B:

  • Bleeding occurred in 12.2% for procedures and 7.4% for teeth.
  • First bleeding occurred at a mean of day 5.3 post-extraction, highlighting the importance of delayed follow-up.
  • Extraction of equal to or more than 4 teeth was associated with higher bleeding risk.

Tranexamic acid use was associated with lower bleeding at both procedure level (adjusted OR 0.43) and tooth level (adjusted OR 0.38).

Interestingly, primary wound closure was associated with increased bleeding, a counterintuitive but important finding.

No thrombotic events were documented.

This study reinforces the value of tranexamic acid in dental procedures for haemophilia patients, and highlights the need for extended post-extraction monitoring well beyond the day of the procedure.”
