This is a typical conversation with an old-school consultant.

Hb: 10.5; all coagulation profile normal.

Post op patient with oozing, completely hemodynamically stable.

Request was for fresh whole blood.

(No, we don’t unnecessarily transfuse blood. We follow international guidelines.)

(No, nothing is done on a whim)

Anything out of the ordinary, I make it a point to call and enquire if there was something we missed.

I asked if we should work up for any coagulopathy and switch to components if needed (very politely and subserviently, as Indian Healthcare demands).

Response: We rely too much on Jurassic Era idiocy masked as intuition or Gut feeling.

Patients have not died till now IN FRONT of us due to blood transfusion (which is not a justification).

How do you handle such scenarios?
