Federico Ravaioli: 40 kPa Spleen Stiffness May Help Avoid Endoscopy in Chronic PVT
Federico Ravaioli, Tenure-Track Researcher (Law 79/2022) at the University of Bologna, shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in Journal of Hepatology, adding:
“SSM 40 kPa may be the number to remember also in chronic portal vein thrombosis.
Our new multicentre study in the Journal of Hepatology suggests that spleen stiffness measurement by VCTE less than or equal to 40 kPa can identify patients with chronic portal vein thrombosis without cirrhosis who have a very low probability of high-risk varices.
In 346 patients from 16 VALDIG centres:
- NPV: 97% in the derivation cohort
- NPV: 96% in the validation cohort
- 41–43% of screening endoscopies potentially spared
- Less than or equal to 5% of high-risk varices missed
This is particularly relevant because, unlike in cirrhosis, liver stiffness cut-offs cannot simply be transferred to chronic PVT, and current strategies still rely heavily on repeated endoscopic screening.
A simple non-invasive measurement may therefore help us better select who really needs endoscopy.
