Jesus Ardila Novoa: How Patient Education Can Transform Hemophilia Care

Jesus Ardila Novoa, Pediatric Hematologist and Oncologist at Club Noel Children’s Hospital Foundation, shared a post on LinkedIn:

”We have been telling patients: ‘Your blood does not clot.’

That sentence is technically correct. It is also one of the most clinically counterproductive things we say.

Here is a communication framework I have been developing — grounded in the cell-based model of hemostasis by Hoffman & Monroe — and why I believe it changes what patients do with their disease.

The vascular wall is not a passive pipe.

When injured, its inner lining exposes tissue factor — the biological alarm that recruits the entire hemostatic response.

The platelets arrive first: fast, in number, forming the initial seal.

Then come the specialized reinforcements — the coagulation factors — who consolidate the closure and guard the repair.

In hemophilia: the alarm fires.

The first responders arrive on time and in sufficient numbers. The reinforcements never come. Not because of urgency. Because the patient was born without that link.

When a patient understands this — not just ‘my blood doesn’t clot,’ but where exactly the system fails — something changes clinically:

They ask for their factor level before a scheduled procedure. They tell the on-call physician which link is missing, not just that they have ‘a bleeding disorder.’ They recognize early signs that the initial seal is failing. They become active safety monitors of their own perioperative care.

Patient education is not a soft skill added at the end of the consultation.

It is a clinical intervention with measurable outcomes.

The language we choose when explaining hemostasis either builds or undermines a patient’s capacity to advocate for themselves — especially in settings where a hemophilia specialist is not in the room.

I am curious: how do you explain hemostasis to your patients?”
