Ahmed Boukaraa: Transfusion Safety – Have You Checked Calcium Levels before Transfusing?

Ahmed Boukaraa, Public Health Biologist – Clinical Biochemistry and Emergency Operations at Hospital of Houari Boumediene in Sedrata, shared a post on LinkedIn:

“Transfusion Safety: Have you checked calcium levels before transfusing?

During a blood transfusion – especially in emergency settings or massive transfusions – attention naturally focuses on ABO/Rh compatibility and infectious screening.

However, a critical metabolic and hemostatic factor is sometimes overlooked: ionized calcium (Ca2+ ).

Why is calcium critical?

  • The Citrate Trap: Blood bags contain sodium citrate as an anticoagulant. When infused rapidly or given to vulnerable patients, citrate binds to the patient’s ionized calcium, forming an inactive complex.
  • The Hemorrhagic Risk: Ionized calcium serves as Factor IV in the coagulation cascade. Severe hypocalcemia impairs clot formation and platelet activation, which can worsen active bleeding.
  • The Cardiovascular Risk: A sudden drop in Ca2+ exposes the patient to refractory hypotension, arrhythmias, and cardiovascular collapse.

Best Practices at the Bedside and Laboratory:

  • Measure Ca2+ via Blood Gas Analysis (ABG): This is the only reliable measurement in acute settings (total calcium does not reflect the chelated fraction).
  • Alert Threshold (Ca2+ below 0.9 mmol/L): Prompt calcium supplementation (e.g., calcium gluconate) is essential to restore hemostasis and hemodynamic stability.
  • The Golden Rule: Always administer replacement calcium through a separate IV line to prevent direct blood coagulation in the tubing.

Transfusion safety doesn’t stop at blood typing : monitoring and anticipating citrate toxicity is vital to saving lives in the ICU and operating room.

How does your facility handle calcium monitoring during massive transfusions?”
