Jens Eschen: Randomized Controlled Trial on Acute Aortic Dissection Anticoagulation Strategies
Jens Eschen, Researcher at Aarhus University Hospital, shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in JTH, adding:
“Grateful that our randomized controlled trial in acute aortic dissection has finally been published in the Journal of Thrombosis and Haemostasis (JTH) of the International Society on Thrombosis and Haemostasis (ISTH).
Acute type A aortic dissection is a life-threatening, time-critical condition that requires emergency surgery.
Many patients arrive with severely impaired blood coagulation, and perioperative bleeding and transfusion of blood products remain major challenges.
In this study, we compared two established anticoagulation strategies during cardiopulmonary bypass: an individualized, patient-tailored approach vs. a conventional weight-based regimen.
Does tailoring improve anticoagulation control, and does it change patient outcomes?
In short: neither strategy adequately suppressed coagulation activation during surgery.
The individualized approach did not outperform weight-based dosing, and bleeding, transfusion and clinical outcomes were comparable.
Our findings point to a need for new anticoagulation strategies in acute aortic dissection.
A big thank you to the study team and above all to the patients and their families who made this trial possible.”
