Aryabhatta Sadhu: Sickle Cell Crisis Recognising When Treatment Must Change

Aryabhatta Sadhu, Attending Consultant and Head of Transfusion Medicine at Fortis Hospital Shalimar Bagh, New Delhi, Member of AABB, Member International Society of Blood Transfusion (ISBT), shared a post on LinkedIn:

“I will be speaking at the upcoming ISBT Webinar, ‘Sickle Cell Crisis: When Diagnostics Should Change Therapy’, alongside Richard R. Gammon (Moffitt Cancer Center; Chair, ISBT Clinical Transfusion Working Party).

A vaso occlusive crisis can become a multisystem emergency within hours.

Progressive hypoxaemia, new neurological signs, falling platelets, accelerating haemolysis, and hepatic or renal injury change what the patient needs, and when.

This case based session sets out a diagnostic to therapeutic framework:

  • Red flags that redefine a vaso occlusive crisis
  • Matching findings to the right strategy: supportive care, simple transfusion or urgent automated red cell exchange, while avoiding hyperviscosity
  • Optimising RCE: patient selection, Hb HbS targets, red cell selection and antigen matching, monitoring and response assessment
  • Sequential plasma exchange after RCE in the uncommon patient who keeps deteriorating, with a clear account of its limited evidence base and why it is not first line therapy
  • A multidisciplinary escalation algorithm with response markers, endpoints and stop rules

Moderated by Ratti Ram Sharma and Sangeeta Pahuja Sindhwani (Lady Hardinge Medical College, New Delhi; Co Chairs, Haemoglobinopathies Subgroup, ISBT Clinical Transfusion Working Party).
