Abdul Muqtadir Abbasi: Anticoagulation in DVT Patients With Hidden Varices Requires Extreme Caution
Abdul Muqtadir Abbasi, Assistant Consultant at King Faisal Specialist Hospital and Research Center, shared a post on LinkedIn:
“Patient came in with a lower limb swelling, US confirms a dvt. You want to prescribe LMWH. Pause!!!
Always make sure patient has no possibility of varices.
A lot of CLD patients and others with relevant disorders, may have undiagnosed varices. Some will even have known gastro esophageal varices.
If these patients started on anticoagulation, can lead bleed to death scenario. So be very careful.
Starting anticoagulation in patients with known varices is a crime.
If needed, get an urgent banding done and then can start in a week or two once banding is stable.
Also avoid DOACS like rivaroxiban, apixaban as they have high GI bleeding tendency.
Please be very careful about this.”
