Remember the 30–50–100 Rule
30
- Ferritin ≤30 ng/mL
- Think Iron Deficiency
50
- Ferritin ≤50 ng/mL
- Consider iron deficiency when symptoms or ongoing risk factors are present
100
With Inflammation
Ferritin <100 ng/mL Or Tsat <20% may support iron deficiency
Easy mnemonic
30 – Deficiency
50 – Symptoms / Risk
100 – Inflammation
20 – Tsat
Think 30–50–100–20
One key message
A ‘normal’ ferritin does Not always mean normal iron stores.
Ferritin is an acute-phase reactant.
Always interpret it with
- Tsat
- Crp
- Hb and Mcv
Clinical context
For anemia
Men – Hb <13 g/dL
Non-pregnant women – Hb <12 g/dL
The clinical question should not be
‘Is ferritin within the laboratory reference range?’
It should be
‘Does this patient have enough available iron for normal erythropoiesis?'”
