“When Transfusion Is Not an Option: HBOT for Exceptional Blood Loss Anemia
A patient bleeds, hemoglobin collapses, and packed red cells are the first-line answer.
Sometimes they cannot be given.
Religious objection, antibodies that make crossmatch impossible, or a delayed blood bank leave a gap.
Tissues still need oxygen while hematinics rebuild red cell mass.
What Exceptional Blood Loss Anemia Means
Clinicians use this term for severe anemia in which hemoglobin can no longer meet tissue oxygen demand. UHMS notes delivery becomes problematic near 6 g/dL and clearly inadequate below about 3.6 g/dL.
Tachycardia, dyspnea, chest pain, confusion, and rising lactate are the clinical flags.
HBOT is not a substitute for stopping hemorrhage or for transfusion when transfusion is available and accepted. It is a time-limited bridge.
Why Hyperbaric Oxygen Helps
Most oxygen rides on hemoglobin.
Under pressure, far more oxygen dissolves in plasma.
At about 3 ATA of 100 percent oxygen, dissolved plasma oxygen can approach the 5-6 mL per 100 mL that tissues typically extract.
Boerema’s classic ‘life without blood’ work showed that dissolved oxygen can keep tissues alive for a short window even when hemoglobin is nearly gone.
OxygenWell treats in grounded monoplace chambers with 100 percent medical-grade oxygen up to 2.4 ATA.
Sessions are pulsed while iron, B12, folate, and other hematinics raise the baseline.
Case reports (including Jehovah’s Witness patients) describe HBOT as an adjunct while hemoglobin recovers.
There is no large randomized trial; ethics make that trial nearly impossible.
UHMS still lists severe anemia as an accepted indication.
Insurance Reality
Medicare NCD 20.29 currently lists exceptional blood loss anemia among noncovered conditions.
Some commercial PPO policies still cover severe anemia when transfusion is refused or not possible.

Do not assume coverage either way.
