Elias Lee։ Beyond MCA Stenosis – Recognizing Moyamoya
Elias Lee, Professor at Nambu University, shared a post on LinkedIn:
“Moyamoya does not always announce itself as a stroke.
Sometimes it looks like migraine.
A 47-year-old man presented with recurrent headache, photophobia, phonophobia and nausea. Neurologic examination was unremarkable.
The turning point was imaging.
MRI demonstrated abnormal vascular structures along the left Sylvian fissure, while TOF-MRA showed severe proximal left M1 stenosis with reduced distal MCA visualization.
The important finding was not simply ‘MCA stenosis.’
It was:
- Stenosis plus collateralization.
That combination should immediately raise suspicion for moyamoya-pattern vasculopathy.
The next question is even more important:
- Is the brain adequately perfused?
Modern moyamoya imaging increasingly integrates MRI/MRA, DSA, perfusion imaging, cerebrovascular reserve, vessel-wall MRI, and longitudinal assessment.
AI may eventually help detect these complex patterns, but it should remain a decision-support technology with expert radiologist verification.
The lesson for clinical AI is equally important:
Detecting an abnormality is not the same as understanding its clinical meaning.”
