Fahad Laghari: Flow Diversion for a Cavernous Internal Carotid Artery Aneurysm

“Many times in my practice, I am grateful for the people who envisioned and pioneered the technologies we use today.
Flow diversion is one of them.
A young patient presented with a week-long worsening and unbearable headache, left periorbital swelling and pressure, blurry and double vision, and a left CN VI palsy.
CT head was negative for hemorrhage, while CTA raised concern for an ICA aneurysm. DSA showed no carotid-cavernous fistula, but did show a 1.8 cm left cavernous ICA aneurysm.
We treated the aneurysm with a Surpass Elite flow diverter.
There was immediate stasis and marked reduction of flow within the aneurysm, but thankfully slow but continued filling.
Immediate thrombosis was not the goal. I wanted slow involution of the aneurysm and gradual relief of pressure on the abducens nerve.
Thankfully this patient is living in a time where they did not need carotid ligation, bypass, clipping, or even … stent-assisted coiling.
Coiling could treat the aneurysm but still leave the patient with mass effect from a packed aneurysm.
Mass effect is what was causing the symptoms.
The patient’s visual symptoms improved immediately after the procedure.
Flow diversion is a technology worth being grateful for.”
