Mohamed Adel: The Warning Signs of Increased Intracranial Pressure

“Day 31 | Increased Intracranial Pressure (ICP)

A headache can be simple… but when intracranial pressure rises, every minute matters.

Today, I focused on Increased Intracranial Pressure (ICP) — a serious neurological condition that can compromise cerebral perfusion and lead to permanent brain injury if not recognized and managed early.

Key Signs and Symptoms:

  • Decreased level of consciousness
  • Headache, nausea and vomiting
  • Pupillary changes
  • Restlessness, confusion and seizures
  • Abnormal posturing

Cushing’s Triad:

A late and concerning sign of increased ICP:

  • Hypertension with widened pulse pressure
  • Bradycardia
  • Irregular respirations

Nursing Priorities:

  • Frequent neurological assessment
  • Maintain adequate oxygenation and ventilation
  • Elevate the head of the bed when appropriate
  • Keep the head and neck in neutral alignment
  • Minimize unnecessary stimulation
  • Monitor for signs of brain herniation
  • Administer prescribed therapies such as mannitol or hypertonic saline

Clinical Pearl:

Increased ICP – decreased cerebral perfusion – increased risk of brain injury.

That’s why a small change in GCS, pupils, or neurological status should never be ignored.
