”A Routine Crossmatch, But Never Just a Routine Job

Today in the blood bank, I worked on an interesting transfusion case.

The patient’s blood group was AB Positive, and an AB Negative red cell unit was selected for transfusion. Although the ABO groups were compatible, we still needed to confirm that the selected donor unit was serologically compatible with the patient.

That’s where crossmatching becomes important.

Crossmatch performed:
Gel Card / Column Agglutination Technique – AHG phase

The patient’s plasma was tested against the donor red cells. After the required incubation and centrifugation steps, the reaction pattern in the gel column was carefully observed.

Result: Compatible

There was no significant agglutination observed, with the red cells forming a compact cell button at the bottom of the microtube.

Why is crossmatching important?

Blood grouping tells us the patient’s ABO and Rh(D) type, but it doesn’t tell the complete story. A patient may have clinically significant antibodies against red-cell antigens that are not detected simply by ABO/Rh typing.

Crossmatching helps us check whether the specific donor red cells selected for that patient are compatible before transfusion.

For me, this is one of the most important parts of blood bank work. A small gel card may look simple, but behind every result there is careful testing, observation, documentation, and responsibility.

Every compatible unit is more than a laboratory result—it is one step toward a safe transfusion.”
