N. Mariana S. Vasquez: Antiplatelets vs Anticoagulants – Key Clinical Differences

N. Mariana S. Vasquez, IMG Physician, shared a post onĀ X:

“Antiplatelets vs anticoagulants

Both prevent thrombosis, but they hit different steps of clot formation.

Antiplatelets

  • Target: Platelet activation and aggregation
  • Examples: Aspirin (irreversible COX-1 inhibitor leads to decreased TXA2), clopidogrel (P2Y12 ADP receptor blocker)
  • Classic uses: ACS, coronary stents, secondary prevention of noncardioembolic ischemic stroke

Anticoagulants

  • Target: Coagulation factors lead to decreased fibrin formation
  • Examples: Heparin (activates antithrombin), warfarin (vitamin K antagonist leads to decreased II, VII, IX, X), apixaban (factor Xa inhibitor)
  • Classic uses: DVT, PE, stroke prevention in AFib

USMLE memory trick

  • Platelet-rich ‘white’ arterial clot treated with antiplatelets
  • Fibrin-rich ‘red’ venous clot treated with anticoagulants

High-yield extras

Heparin requires monitoring aPTT, reverse with protamine Warfarin requires monitoring PT/INR, reverse with vitamin K plus PCC.

Key distinction: This is a pattern, not an absolute rule. Treatment depends on the cause of thrombosis and the clinical indication.”
