Paul Agwu: A Low Platelet Count Is Not a Diagnosis – It Is a Clue

Paul Agwu, Head of Department at Garki Hospital, shared a post on LinkedIn:

“A Low Platelet Count Is Not a Diagnosis. It Is a Clue.

Thrombocytopenia is one of the commonest abnormalities encountered in clinical practice. Yet, the number alone rarely tells the whole story.

Before attributing it to infection, prescribing treatment, or initiating an extensive investigation, ask a few important questions:

Is the thrombocytopenia real?

Exclude pseudothrombocytopenia, particularly when platelet clumping is suspected.

Is it isolated or part of a broader haematological abnormality?

Examine the other cell lines. Anaemia, leucopenia or abnormal circulating cells may change the entire diagnostic approach.

What does the peripheral blood film show?

Platelet morphology, red cell fragmentation, abnormal white cells and other morphological findings may provide important diagnostic clues.

What is the clinical context?

Consider the medication history, infections, liver disease, immune-mediated destruction, consumption and possible bone marrow pathology.

The platelet count tells you the extent of the abnormality. The clinical context, blood film and targeted investigations help determine its cause.
The difference between treating a laboratory result and treating a patient lies in the quality of the diagnostic reasoning.

In haematology, we don’t just count cells. We interpret what they mean.”
