Nahush Chafekar: Anticoagulation Strategies in Haemodialysis Patients with AF

Nahush Chafekar, Senior Clinical Fellow in Renal Medicine at the Royal Wolverhampton NHS Trust, shared a post on LinkedIn:

“Happy to have had the opportunity to present our abstract at the American Society of Nephrology (ASN) 2026 Kidney Week held in Denver, Colorado.

Our study was titled ‘Anticoagulation Strategies in Patients on Haemodialysis with Nonvalvular Atrial Fibrillation: A Two-Centre Real-World Analysis’.

It compared the use of Warfarin Vs DOAC Vs No anticogulation in patients with Non Valvular Atrial Fibrillation having End Stage Renal Disease on Haemodialysis.

Conclusions

  1. No anticoagulation demonstrated BEST risk-benefit profile across all outcomes (hypothesis-generating)
  2. DOAC showed dramatically elevated bleeding risk (57% CRNM, p equals 0.061 vs warfarin) without stroke benefit
  3. Warfarin showed poorest ischemic protection (9.1% stroke rate in largest group)
  4. Antiplatelets dramatically WORSEN outcomes (5 times stroke increase, 6 times bleeding increase)
  5. Current UKKA guidance stating ‘equivalence’ of all strategies is challenged by these data

Thanks to Sujay Iyer for the efforts taken!”
