”Bempedoic Acid and Venous Thromboembolism Risk in Statin-Intolerant Patients

In this post hoc analysis of the CLEAR Outcomes randomized clinical trial, investigators evaluated whether bempedoic acid reduces the risk of venous thromboembolism (VTE) in statin-intolerant patients at high cardiovascular risk.

A total of 13,970 participants (mean age 65.5 years; 48.2% women) were randomized to bempedoic acid 180 mg daily or placebo and followed for a median of 40.6 months.

During follow-up, 106 VTE events occurred.

Bempedoic acid was associated with a significant 42% relative reduction in VTE compared with placebo (39 vs 67 events; HR 0.58, 95% CI 0.39–0.86; P=0.006).

Consistent benefits were observed for both major components of VTE: deep vein thrombosis (HR 0.56, 95% CI 0.31–0.996; P=0.045) and pulmonary embolism (HR 0.61, 95% CI 0.37–0.996; P=0.046).

These findings extend previous evidence suggesting that lipid-lowering therapies may provide protection beyond atherosclerotic cardiovascular disease, potentially reducing thrombotic events through anti-inflammatory effects, lipoprotein(a) reduction, and enhanced clearance of coagulation factors.

Importantly, the observed reduction in VTE persisted even after accounting for major cardiovascular events.

Although limited by its post hoc design and the relatively small number of VTE events, this analysis suggests that bempedoic acid may offer an additional clinical benefit in statin-intolerant patients by lowering both cardiovascular and venous thromboembolic risk.

These results should be considered hypothesis-generating and warrant prospective confirmation.”
