A valuable review from Hemostasis Today highlighting an important aspect of Polycythemia Vera management: thrombosis prevention requires more than simply treating the acute event.
PV is a chronic myeloproliferative neoplasm associated with a significant risk of both arterial and venous thrombosis.
Key clinical takeaways:
- Maintaining hematocrit <45% remains a cornerstone of thrombosis prevention.
- Following VTE, anticoagulation should be individualized according to the clinical scenario and patient-specific bleeding/thrombotic risks.
- The need for concomitant aspirin should be carefully reassessed when therapeutic anticoagulation is initiated.
- Appropriate cytoreductive therapy plays an important role in high-risk patients and secondary prevention.
