”Dengue Management: Watch the Hematocrit (Hct), Not Just the Platelets
Thrombocytopenia is common, but plasma leakage is what turns dengue life-threatening.
Why serial Hematocrit is your most critical bedside marker:
The Capillary Leak Window:
- A rising Hct indicates hemoconcentration due to active plasma leakage.
- A greater than 20 percent increase defines severe plasma leakage.
The Red Flag Divergence:
- Rapidly falling platelets combined with a rising Hct suggests active capillary leakage and requires hospital admission and prompt initiation of intravenous crystalloids.
Fluid Titration Guide:
Rising Hct with unstable vital signs:
Increase intravenous fluid resuscitation.
Falling Hct with stable vital signs:
Gradually reduce intravenous fluids to prevent iatrogenic pulmonary edema.
Falling Hct with worsening shock:
Consider occult hemorrhage and transfuse blood rather than administering additional crystalloids.
Platelets show the diagnosis; hematocrit guides fluid resuscitation.”
